Use exactly as your healthcare provider tells you. Take the tablet with or without food and swallow with a glass of water. If you miss a dose, take it as soon as you remember unless it is almost time for your next dose. Do not take two doses at the same time.

Tadalafil blocks the PDE-5 enzyme in blood vessel walls. This keeps more of a natural chemical called cGMP around, which relaxes smooth muscle and lets more blood flow into the penis. For BPH, the same muscle-relaxing action eases pressure in the prostate and bladder. You may notice better erections in about 30–60 minutes, and the effect can last up to 36 hours. Relief of urinary symptoms may take several weeks of daily use.

Do not use if you take nitrates (like nitroglycerin) or guanylate cyclase stimulators (such as riociguat); the combination can cause dangerous drops in blood pressure. Do not use if you have severe heart disease and your doctor has told you to avoid sexual activity. Do not use if you have had a recent heart attack or stroke within the past 90 days, uncontrolled high or low blood pressure, or severe liver disease. Do not use if you are allergic to tadalafil or any ingredients in this product. Not approved for anyone under 18 years old.

Serious interaction with any nitrate medication (Nitrostat, Imdur, isosorbide) or recreational "poppers" containing amyl nitrate. Use caution with alpha-blockers for prostate or blood pressure (tamsulosin, doxazosin) because of dizziness risk—your doctor may adjust timing or dose. Strong CYP3A4 inhibitors like ketoconazole, itraconazole, clarithromycin, ritonavir, and grapefruit juice can raise tadalafil levels and side effects; dose changes may be needed. Rifampin, carbamazepine, and St. John’s wort can lower tadalafil effectiveness. Limit alcohol to small amounts; heavy drinking can increase side effects such as fainting.

Very common (more than 1 in 10): headache. Common (1–10 in 100): flushing, upset stomach, back pain, muscle aches, stuffy or runny nose. These usually fade within a few hours. Uncommon (1–10 in 1,000): dizziness, blurred vision, swelling of eyelids. Rare but serious: prolonged erection lasting more than 4 hours (priapism), sudden vision loss in one or both eyes (NAION), sudden hearing decrease or loss, severe allergic reaction with rash and swelling. Call your doctor right away for chest pain, fainting, or any serious or lasting side effect.

Store at room temperature 68-77°F (20-25°C). Keep tablets in the original, tightly closed container, away from moisture, heat, and direct light. Do not store in the bathroom or near a kitchen sink. Keep out of reach of children and pets. Dispose of unused or expired tablets through a pharmacy take-back program or mix them with used coffee grounds in a sealed plastic bag before placing in household trash.
